You are ready to get assessed! In order to do that, it will require the participation of your physician to get your medical records. A full assessment requires a medical assessment as well as observation by a trained therapist.

The form below is a referral form from your Physician. It is in PDF format, so you can print it out and bring it to your physician to complete it, sign it and send it to us.

Keep in mind that to get you back on the road, provided it is possible, will require your active participation as well as that of your physician and the government office in charge of licensing. It will likely also require the participation of your insurance company. In B.C. that might include ICBC or Worksafe BC.
